Before you get started:

You will need to have an NDIS number.

If you are not registered with the NDIS and you would like to access the National Disability Scheme please call NDIA on 1800 800 110.

You will need to know when your plan starts and ends.

This information will be given to you by the NDIA when registering. If you are unsure, please contact the NDIA on 1800 800 110.

You will need to know how/where your funds are allocated.

This information will be given to you by the NDIA when registering. If you are unsure, please contact the NDIA on 1800 800 110.

If you are Plan-Managed:

This is when a third party and a plan manager is looking after, assisting you and allocating your funds for you. Your planner will be invoiced directly by us and they will process this through the myplace portal on your behalf so that your invoice is paid. You will need to check if you have sufficient funds allocated to the following categories:

For exercise physiology you will need to have Capacity Building (CB) funding under “Improved Health & Wellbeing” and/or “Improved Daily Living Skills”.
For physiotherapy and osteopathy, you need Capacity Building (CB) funding under "Improved Daily Living"

If you are self-managed:

This is when you are managing and allocating your own funding. If you are self-managed you will be invoiced by your provider (us) and you pay us directly. You are then provided with a receipt of payment which you claim against your plan using the myplace participant portal. You can choose how much of your funds are allocated to our services; exercise physiology, physiotherapy or osteopathy. We have the appropriate requirements for self-management; ie – we are an NDIS registered provider with an ABN number.

If you are NDIA-managed:

This is when we, as a registered provider, submit a payment request through the myplace provider portal to receive payment for the services that you attend with us. This is done on your behalf, with your approval. This is only available for exercise physiology services. To allow us to do this we need the following information from you:

Your full name as recognised by the NDIA

Your reference number or NDIS number

You have all the above information, now what?

Request a quote.

This will include an itemised list of the services that we are quoted for and exactly how much each service will cost you so that you know exactly where your funds are going.

Call us on 1300 920 520 to request a quote or fill in the form above.

We prepare a service agreement for you.

This service agreement will need to be completed before starting your chosen services with us and it outlines the following:

We will then get you booked to start!

Your first appointment with us is a one on one health assessment were we will discuss your condition/s, any difficulties you may encounter, your previous exercise/medical history, create goals that are inline with your NDIS plan and do some baseline exercise testing. This will help with creating a meaningful plan moving forward to assist you with reaching your goals.

All of your services with us will need to be pre-booked and we will discuss this in more detail when booking. If at anytime you have any questions or need to make any changes please call on 1300 920 520 or speak to us in person when you are next in for a visit. We are looking forward to helping you.
